The following services- Discharge Planning & Transfers, Patient and Family Education
Cultural Awareness, and Ethics Committee are provided by our professional skilled team of health care workers.
Nursing Care – Our hospice program is unique because it focuses on the whole person and also cares for the emotional and spiritual needs of the families. Our nurses are trained in Pain and Symptom Management. The nurse's focus is on palliative care.
Psychosocial Services – Provided by skilled professionals who are able to assess what is most helpful and beneficial to both the patient and family members. By recognizing the individuality of each family; the social workers counsel, reinforce strengths of family members, assist with day to day coping, other issues and concerns. They also provide direction regarding helpful community resources.
The Bereavement Services Department – HospiceCare of Southeast Florida, Inc., provides support and counseling for our families during this time of grief.
The loss of a loved one has many meanings for us. While every person's experience is unique, there are some common features of grief that we all share. Hospice offers the following programs to help the family make their journey to recovery.
- Individual Counseling
- Camp Coral (A Bereavement Camp for Children)
- Community Outreach Programs
- Support Groups
These teams of caring professionals are available for personal visits or will help make arrangements for visits from your traditional spiritual support system. Hospices are non-denominational and have no particular religious affiliation.
Nurses Aides – Provide direct personal care under the supervision of a registered nurse. Their services include bathing, skin care, assisting with activities of daily living, preparing light meals and light housekeeping tasks that relate to patient care.
Patient Care Volunteers – Available to our patients and families on request to provide companionship, run errands and/or provide respite time for family members. They receive many hours of training. If you are interested please let your team members know and arrangements will be made for you.
Medical Equipment and Supplies – provided under the hospice plan such as wheel chairs, bedside commodes, walkers, hospital beds and oxygen. If necessary we also provide bandages and other medical supplies needed for your care. Safety instructions are given on all equipment provided.
Medications – Provided for pain and symptom management under the direction of your physician. The nurse will instruct on the administration, dosage, side effects and safety issues regarding storing these medications. Medicare pays for all medications related to the terminal diagnosis.
24 Hour On-Call Nursing Staff – Available 7 days a week, including holidays. Listed below are some of the reasons why you should call the triage nurse after hours, on the weekends or during the holidays:
At times, it may be better to call us in order for you and your family to have peace of mind about your concerns.
- Uncontrolled pain
- Difficult breathing
- Nausea and/or vomiting
- Seizures
- Falls
- Extreme agitation or confusion
- Fever of 101 or greater
- Inability to urinate or problems with a foley catheter
- Loss of consciousness
- Death
